A brief description of each form is
available below.
WWA Membership Form
Each participant is required to
purchase WWA insurance to
participate in AWA sponsored events.
There are two membership
options; single event and full year. If an individual
has purchased WWA Full Year
Membership online; we ask that you
bring your membership card or copy
of online receipt. This form is only filled
out once per season, unless the
participant chooses to purchase a
single event membership in which
case it must be filled out for every
single event purchased. This form
can be printed and turned in at the
registration booth during
registration hours. This form will
also be available to fill out at
each AWA sponsored event.
WWA Liability Waiver
This form must be signed by each
participant.
Participants under the age
of 18 must have this form signed by
his/her parent or legal guardian.
Participants under the age of 18 who
do not have this formed signed by
his/her parent or legal guardian
will not be permitted to ride at the
event. This form can be printed and
turned in at the registration booth
during registration hours. This form
will also be available to fill out
at each AWA sponsored event.
AWA Rider Profile
This form enables the announcer to
better talk about you during your
run. This is not a required form;
however we encourage each rider to
complete it. This form can be
printed and turned in at the
registration booth during
registration hours. This form will
also be available to fill out at
each AWA sponsored event.
